Manhattan New School P.S. 290 2024-25 Drop-Off Event WAIVER for Class of 2026 Graduation Party
· I hereby grant permission for the child(ren) specified on the event registration page to participate in the MNS/PTA Class of 2026 Graduation Party on Friday, June 20 2026 (the “EVENT”) at MNS in the Cafe located at 311 E 82nd Street NY NY 10028, and affirm that my child(ren) is capable of participating in and has permission to engage in all activities at the EVENT. I, or the Emergency Contact listed on the event registration page, will be reachable by phone at the contact number specified for the duration of the EVENT.
· I hereby release and hold harmless Manhattan New School PS 290, MNS/PTA, Inc. and their respective employees, officers/board members, volunteers and other representatives from any accidents, diseases, losses, liabilities, damages or injuries related to attendance at or participation by the child(ren) specified on the event registration page in the EVENT. The participant hereby further agrees not to sue MNS/PTA, Inc. for the claims waived and released in this Agreement.
· Should my child(ren) become ill or injured, I hereby authorize the volunteers at the event to seek medical care appropriate to my child(ren)’s injury/illness. Further, the Participant agrees to pay for all costs associated with medical care and transportation for the Participant.
· I further understand and agree that should my child(ren) act in a way that is dangerous or disruptive, they may be removed from the EVENT and could be barred from future MNS PTA events.
